Bilingual (English/Spanish) Diabetes Prevention Coordinator

CHS seeks a bilingual (English/Spanish) Diabetes Prevention Coordinator to help develop the infrastructure for local diabetes prevention lifestyle programs, promote the program to English- and Spanish-dominant populations and support lifestyle coaches with program implementation for English- and Spanish-dominant populations in Southern New Jersey (Cumberland, Gloucester, and Salem Counties).

Job Responsibilities: 
  • Establishing relationships with public health, physician, public and private payers, and other referral networks to enhance awareness of and referrals to the lifestyle program
  • Serving as a liaison, ambassador, and advocate for the lifestyle program within public health, physician, health care professional, and payer communities
  • Responding to inquiries about the lifestyle program from the general public and members of the public health, physician, health care provider, and payer communities
  • Collaborating with the Program Director to leverage existing relationships with public health, physician, health care provider, and payer communities to benefit the lifestyle program
  • Acting as a spokesperson for the lifestyle program to local media
  • Supporting lifestyle coaches in implementing the lifestyle program
  • Recruiting, screening, and registering eligible participants into the lifestyle program
  • Organizing a master schedule of the lifestyle program classes
  • Ensuring adequate publicity for and marketing of the lifestyle program
  • Assisting lifestyle coaches with launching each group
  • Assisting with retaining lifestyle program participants
  • Providing class coverage, if needed to prevent cancellations
  • Ensuring compliance with all applicable laws and regulations, including those governing privacy and data security
Job Requirements: 
  • Bachelor’s degree in a related field
  • Ability to build a network of referrers
  • Ability to act as a resource for lifestyle coaches by answering questions and providing evidence-based information in a timely manner
  • Familiarity with the local public health community
  • Must role model a healthy lifestyle
  • Ability to administer all aspects of the lifestyle intervention delivery to include:
    • Experience in health education or behavior modification and group facilitation or training
    • Experience delivering facilitation that optimizes social interaction, shared learning, and group cohesion
    • Understanding of and experience overseeing participant safety-related issues related to program delivery
    • Ability to guide behavior change efforts in others without prescribing personal actions or solutions, so that participants increase their self-confidence and capacity to make and sustain positive lifestyle changes
    • Ability to empathize with participants who will likely experience difficulty and frustration with trying to adopt and sustain healthy lifestyle behavior changes and who may differ from the lifestyle coach in terms of weight status and level of commitment to living a healthy lifestyle
    • Ability to build strong relationships with individuals and build community within a group
    • Knowledge of basic health, nutrition, and fitness principles
    • Knowledge of behavior change principles
    • Experience using motivational interviewing techniques
    • Flexibility to work with people from all walks of life
  • Strong interpersonal and communication skills, verbal and written and in English and Spanish
  • Attention to detail and keen organizational skills
  • Data collection experience
  • Ability to work independently and effectively establish priorities
  • Ability to maintain integrity of confidential information
